Why do I keep breaking out?

I just got over my period a couple of days ago and I was ovulating so I thought that it were my hormones BUT MY SKIN KEEPS GETTING NEW ACNE! I also got a new moisturizer that I thought was causing my acne but I don't even know anymore. It's irritating because my skin was really clear, but then as soon as I go my period my skin will not clear up. I cannot get a break from a breakout!
